I am unable to fill out the form to order a free sample. Everytime I choose the correct state, the form resets itself.

You have to be patient. It is running really slow… probably a lot of people requesting one.

Site not working properly. As stated by Megan when you enter the state form resets it’s self and you no lomger can inter info.
