The link brings you to a site called start sampling. Does anyone know anything about this site and if it’s worth doing?
I’ve used Start Sampling before – it’s definately worth doing. I’ve never had problems with them. (A lot of companies, especially P&G, use them to get their samples out there and promote their products.) However, when I go to get my free sample from them it says “URL Not Found.”

Apparently they are out of samples or their webpage is broken because like Dawn I get URL Not Found

It is working now … I left some feedback on the page when it wasn’t working and it was up and running in 10 minutes!
Apparently you only get the sample if your survey answers indicate that you are at risk for gingivitis. No bleeding gums, no freebie!
At the end of my survey, when it didn’t say it sounded like I had gingivitis, it said I could try a different kind of their toothpaste, but when I clicked on the page to get the sample, it dumped me back to walmart’s main page. Bummer.
Link must be fixed.I was able to fill out survey and get free sample.

I just tried doing what you did and the link to the Crest with Scope Outlast sample popped up. I was able to request one for my mom. I used a different browser (IE instead of FF) than when I requested my sample of Crest Pro Health Clinical Gum Protection toothpaste (plus samples for a bunch of my friends and family—they leave this free sample business up to me haha). Maybe, if you try a different browser, the form will pop up for you. IDK… Just a suggestion.
