Not working for me:( Says this when I submit:

Error. The server encountered an unexpected condition which prevented it from fulfilling the request.

First I got an error message, then it took four or five times submitting it before it said my samples would be sent. I hope it works!

I found if I went through frugal mamas site it worked fine for me, but had lots of trouble trying to do it from here :(

I couldn’t get it to work (kept getting error message) from here but went to Thrifty Frugal Mom’s blog and it worked fine from there.

I got this one already about a month ago and had forgotten that I requested it. It made my day getting a cute and tiny tampon holder with tampons!!
