I homeschool and got one. It has an address for the school you work for, but I just put in my home addy since that is where my school is. I think that is all you need. I haven't used mine yet. All I did was fill out the form and I got a screen that had the card and it just said to print it out. I am not sure what I clicked on to get the form though. But as stated already you do need a "teacher's reward card." They let me do my application over the phone. The gentleman gave me my reward card number to use until I receive my official card. You could always call your stores and ask. Our cover school actually issues us a "teacher id" but I wasn't asked for anything. There is a square on the Teacher's Reward Card form for "homeschool" so I checked it and that was that!
